This notice is to inform users of the new functionality related to the SAB
Forms (A, B, C, & D). Once logged into the system, under the Members
Main Menu Item, you now have the ability to complete the SAB Form online,
by clicking on the link to "Add SAB Form. The following fields will
need to be completed:
Enter the Registration Number of S.A.B, Transfer Approval #, and Date of
Enter the Name of the Deceased:
Select the Race of the Deceased from the drop-down menu:
Select the Sex of the Deceased from the drop-down menu:
Select the Age of the Deceased from the drop-down menu:
Enter the Place of Residence, Place of Death, and Cause of Death:
Select the Body Part Transferred from the drop-down menu:
Enter the Agent of Anatomical Board:
Enter the Address of Agent:
Select the Date the body was received from the pop-up date menu:
Select the Date shipped from the pop-up date menu:
Enter the name of the location (College or Physician), that the body was
Enter the address to which the body was shipped:
Enter the specifics of whether or not the deceased, as far as you know, was
free from any contagious disease.
Select your user name from the drop-down menu under the option for
Once the page loads, if you scroll to the bottom of the page, click on the
“Print” button, and print 4 copies of the SAB Form.
According to the form instructions:
Note for Agent of the Anatomical Board: Keep Form D as you temporary record
and Send Forms A-B-C to the recipient. Form C will be signed and returned
to you as your receipt and permanent record.
Note to Recipient: Check for corrections and sign this FORM A and send to
the Secretary of the Anatomical Board.
